Please note that by booking an appointment with Move With Eze you are agreeing to our Terms & Conditions and our privacy Notice. You also agree to attend the appointment you have booked.

GENERAL CONDUCT:

Appointments with Move With Eze can be made in a number of ways including via our website, telephone, text or email. Appointments are available for a variety of durations from 45 mins to 90 mins.

Once you’ve made a booking, you will receive a confirmation email, along with an email reminder 2 days prior to your appointment.

If this is your first appointment with us, you will have to complete a consultation form prior to treatment and this will be part of your appointment time. This is why we advise you to arrive a few minutes beforehand.

Any inappropriate behaviour (illicit or sexually suggestive remarks, sexual advances, drug use, excessive drinking, etc.) will not be tolerated and will immediately terminate your treatment. You will still be responsible for paying for the session in full.

CANCELLATIONS & LATE ARRIVALS:

Cancellation Policy: 24 hours.

Move With Eze operates a minimum 24-hour cancellation/rescheduling policy. If an appointment is missed, cancelled or changed with less than 24 hours notice, there will be charge of cancellation fee.

Please cancel as soon as possible if a foreseeable event is likely to prevent you from attending. If you need to cancel your appointment at short notice because of a long-term medical condition, which varies from day to day, or because you are more than 37 weeks pregnant, we will discuss the cost implications on a case by case basis.

An invoice for any cancellation or ‘no show’ charges will be sent to you by email for payment by bank transfer within 7 days. Following a cancellation, payment must be received before any further massage bookings can be taken.

 Late Arrivals: It is important that our agreed appointment time and duration is adhered to. We appreciate that sometimes things happen, and you may arrive a little later or need to leave a bit earlier than planned. Please note that we cannot make up the missed time by overrunning or adding time to your original appointment.

 New appointments:  Once we receive a booking request from our booking system, we will email you a confirmation (subject to availability), or offer you alternate dates or times. We do require a £30 booking fee which is redeemable against your treatment providing you attend the appointment or give us 24hrs notice of cancellation or change of time. An email link will be sent for the payment and once this has been paid the booking will be confirmed.

CONFIDENTIALITY:

All client information and clinical records will be safeguarded by Move With Eze and will remain confidential. Unless required to do so by law, we will not otherwise share, sell or distribute any of the information you provide to us without your consent.

By signing the Intake forms, you are giving Move With Eze permission to hold your records and data. Consultations and client treatment details will not be discussed with anyone other than you, unless the client is under 18 or has a care worker or guardian.

Move With Eze will access clients records solely for the purpose of providing you with the most appropriate treatment.

online consultations:

During the online call, details of your medical history, examinations and tests will be discussed through the use of interactive video and audio technology.

A physical assessment may take place. Video, audio recording may be taken of you during the procedure or service. 

Please note, we do not record or keep all telecommunications. In addition, any patient's identifiable images or information for this online interaction shall not occur without their consent nor passed onto any other parties. 

Confidentiality: reasonable and appropriate efforts have been made to eliminate any confidentiality risks associated with online consultations, and all existing confidentiality protections under UK law apply to information disclosed the online consultation

Rights: you may withhold or withdraw your consent to the online consultation at any time without affecting your right to future care or treatment.

Risks, Consequences & Benefits: you have been advised of all the potential risks, consequences and the benefits of online consultations. 

YOU agree to participate in online consultation services above by signing the online terms and conditions form.

Gift vouchers:

Upon payment, a gift voucher with an exclusive number will be emailed to you. Gift vouchers are valid for six months from the date of purchase. The unique gift voucher must be shown before treatment.

DRESS:

Customers are requested to wear a form of underwear that is appropriate to receiving treatment.

HEALTH & SAFETY:

All Move With Eze therapists are fully qualified, hold a First Aid certificate, and have professional and public indemnity insurance for the services that they provide.

Due to the nature of our treatments and the need for close contact, you are asked not to attend for a treatment if you are unwell, suffering from a cold or virus, infection or general ill health.

Please contact us via email at info@movewitheze.com at the earliest opportunity to rearrange your appointment.

Clients under the age of 18 must be accompanied by a parent or guardian who must give permission for the treatment to be performed, and will be requested to co-sign the New Client Consultation Form.

They will be invited to stay in the room for the duration of the appointment, and expected to remain present throughout the treatment.

DISCLAIMER:

Move With Eze provides many forms treatment to help with rehabilitation, however none of these can be considered a substitute for professional medical care or counselling.

Move With Eze will refer any clients to a GP or other professionals if needed.

You are required to provide accurate and true medical and personal information, to the best of your knowledge, and to keep Move With Eze up to date with details of any medication, symptoms, medical concerns, treatments that are being investigated or undergoing treatment for. 

Move With Eze may refuse to treat any client or part of their body with just and reasonable cause if they believe that it is not safe or in the client’s best interest to continue.

The client also has the right to refuse, modify or terminate treatment at any time, regardless of prior consent given.
